چکیده

Diabetic ketoacidosis is a major cause of morbidity and mortality in patients with insulin dependent diabetes. Myocardial infarction is an uncommon but well-recognised precipitating cause of diabetic ketoacidosis, accounting for 1% of cases. Many diabetic patients with ketoacidosis initially present with hyperkelamia, which may affect the electrocardiographic morphology. We present a patient with diabetic ketoacidosis and hyperkalemia, whose initial electrocardiogram showed a sinoventricular rhythm and subsequently pseudoinfarction pattern. (Cardiol J 2007; 14: 497-499).

Clinical and ECG patterns of pseudoinfarction in a young man with type 1 diabetes, diabetic ketoacidosis and normokalaemia

Diabetic ketoacidosis (DKA) can cause changes in the electrocardiogram (ECG) in the form of transient ST-segment depression, QT prolongation, changes in T-wave morphology and the appearance of U wave, possibly due to changes in the serum potassium level. Occasional reports indicate the possibility of transient ST-segment elevation imitating myocardial infarction in the course of hyperkalaemia a...

Pseudoinfarction pattern in a patient with hyperkalemia, diabetic ketoacidosis and normal coronary vessels: a case report

INTRODUCTION A rare electrocardiographic finding of hyperkalemia is ST segment elevation or the so called 'pseudoinfarction' pattern. It has been suggested that hyperkalemia causes the 'pseudoinfarction' pattern not only through its direct myocardial effects, but also through other mechanisms, such as anoxia, acidosis, and coronary artery spasm. CASE PRESENTATION A 33-year-old Caucasian woman...

Severe hypothermia with Osborn waves in diabetic ketoacidosis.

Diabetic ketoacidosis causes substantial morbidity and mortality.1 Though the management of acute diabetic ketoacidosis centers on correcting hyperglycemia and electrolyte abnormalities, other important metabolic derangements may be seen. Specifically, hypothermia may be both a cause and consequence of diabetic ketoacidosis, and the severity of the hypothermia correlates with the severity of th...

PICO-based problem solving (question-answer) of the ST-segment elevation in the aVR lead in a patient with chest pain referred to a hospital emergency department: Letter to Editor

In evidence-based medicine, constructing research questions is the most important part of research integration. Formulating a research question is the first step to conducting a credible and valuable research. Patient, intervention, comparison, outcome (PICO) is a specialized framework used by most researchers to formulate a research question and facilitate resource review. In the heart ward of...

Surgical Extirpation of Apical Left Ventricular Thrombus in Takotsubo Cardiomyopathy

We report a patient with takotsubo cardiomyopathy who underwent surgical resection of apical left ventricular thrombus. A 59-year-old woman was transferred to our hospital in shock with hypothermia and diabetic ketoacidosis. The electrocardiogram showed ST segment elevation, while echocardiography revealed a reduced ejection fraction with apical and midventricular akinesis. Emergency coronary a...
